‘Grey’s Anatomy’ Boss Meg Marinis Breaks Down the Premiere’s Time Jump, Simone’s Pregnancy Bombshell and Jo’s Shocking Exit
‘Grey’s Anatomy’ is back at Grey Sloan Memorial, and the long-running medical drama opened its new season by shaking up nearly every corner of the hospital. Season 23 premiered on ABC on Thursday, October 15, with “Collide,” an episode directed by Debbie Allen.
The premiere threw the doctors straight into chaos. A collision between 2 planes on the Sea-Tac runway flooded the trauma bays on the same day the hospital unveiled a controversial new VIP floor, setting up a season about money, power, and who gets care.
Now showrunner Meg Marinis is unpacking the episode’s biggest swings. Speaking with Deadline, she addressed the premiere’s time jump, Simone’s pregnancy surprise, James Wolk’s arrival, and the doctor who walked away, along with the status of the franchise’s Texas spinoff.
The premiere picks up after a time jump, with Grey Sloan under very different leadership. Owen Hunt (Kevin McKidd) and Teddy Altman (Kim Raver) left at the end of last season, and their exit opened the door for a newcomer.
That newcomer is Seth Norton, played by ‘Mad Men’ and ‘Zoo’ alum James Wolk. Seth is a surgeon-turned-businessman who backs the new VIP floor, and Catherine and Richard name him the hospital’s new chief of surgery. Wolk is set to appear in at least 4 episodes.
Marinis was clear that the show didn’t want to treat Owen and Teddy as easy to replace. “I really didn’t want to say that those two characters or doctors at the hospital were replaceable,” she told Variety.
The premiere closed with a twist for Dr. Simone Griffith (Alexis Floyd), who learns she’s pregnant. The father could be either Lucas or Wes, which pulls her love triangle into even messier territory.

Marinis explained that the news lands at a complicated moment for Simone. “She’s definitely not ready to have a baby, but she doesn’t know if this is the one time that she’s got,” she said, connecting the storyline to a fertility arc from last season.
The doctor’s exit belongs to Jo Wilson (Camilla Luddington), who quits her job by the end of the premiere. The decision is tied to the aftermath of her traumatic birth experience. “PTSD after birth trauma is something that affects so many women,” Marinis said. She told TheWrap that Jo is now reassessing her path: “She’s questioning her career.”
Jo’s departure isn’t the only strain on her marriage. Her husband, Link (Chris Carmack), is secretly relying on painkillers, and Marinis has said the storyline won’t be resolved quickly. Beyond Seattle, the franchise is growing. Marinis is co-creating an untitled spinoff with Shonda Rhimes, set at a medical center in rural West Texas, with an entirely new team of doctors.

The series is expected to premiere midseason in 2027, but nothing is locked in. “The timing on that is still being discussed, just because we want to do what’s best for both shows,” Marinis said. Asked whether she already has a title in mind, she teased, “I have an idea, yes.”
The rest of the ensemble has plenty to deal with too. Meredith (Ellen Pompeo) is skeptical of the VIP floor and predicts it will implode, while Blue (Harry Shum Jr.) refuses to accept being fired and keeps practicing medicine in secret.
With a shorter-than-usual episode order expected and the cast already reshuffled, Season 23 looks set to move fast. Marinis’ comments make clear that Grey Sloan’s biggest shakeups may still be coming.
