‘Grey’s Anatomy’ returns to ABC on Thursday, October 15 at 10 p.m. Eastern. Months have passed since the last episode, and a lot changed on the way out of the previous season. This ‘Grey’s Anatomy’ recap covers what viewers should remember before sitting down again.

The final episode of the previous season aired on May 7, and ABC confirmed another run soon after. The new episodes arrive with two familiar faces gone, several relationships in flux and a hospital that needs new leadership. Here is where everything stands.

How the Season 22 Finale Ending Set Up What Comes Next

The finale was titled “Bridge Over Troubled Water” and broke from the show’s habit of closing on a cataclysmic cliffhanger. It chose a tribute to its two departing stars instead, complete with a montage of their shared history.

The episode’s tension began with a collapse. Teddy learned that a bridge Owen uses to commute had collapsed, and she was left holding a voicemail from him that cut off suddenly. That came after the two had argued over a job offer for Teddy in Paris.

The structure mattered because it left fewer loose ends than usual for the leads, while the supporting characters were left in far messier territory. Some viewers noticed that only a handful of characters appeared in the finale and wondered whether more exits were coming. Showrunner Meg Marinis told Deadline that the audience will always have to wait and see who turns up.

The Owen and Teddy Exit Explained

Owen and Teddy ultimately chose each other over everything else, leaving the hospital together and moving to Paris. The departure played as a ride into the sunset after a season of marital strain and distance. The exits of Kevin McKidd and Kim Raver were made official in late March, and Shonda Rhimes promised the couple the happy ending their story deserved.

The writing leaned on the show’s own history. Teddy’s line about having misread the situation was a callback to a conversation the pair had on a bench outside the hospital in season six. Marinis spoke about the finale’s scale in an interview with TheWrap, where she said she had told people to speak fast.

The practical effect is a structural hole at Grey Sloan. The hospital is now without a trauma attending and a chief of surgery, which TV Guide expects to cause shakeups. Whoever fills that office will inherit a staff with plenty of unresolved tension.

Cast Changes Viewers Should Know About

Ellen Pompeo, Chandra Wilson and James Pickens Jr. are all expected to return. The wider regular cast also includes Jason George, Camilla Luddington, Caterina Scorsone, Chris Carmack, Alexis Floyd, Harry Shum Jr., Adelaide Kane, Niko Terho, Anthony Hill and Trevor Jackson.

One status remains unclear. Scott Speedman, who plays Meredith’s boyfriend Nick Marsh, is a question mark because he is also starring in ‘R.J. Decker’, which airs on ABC this fall. Fans are left wondering whether they will see Meredith and Nick exchange vows at the courthouse.

There is also a new arrival. Deadline reported in early September that James Wolk will play Seth Norton, described as an arrogant surgeon turned businessman who never loses sight of the bottom line. Wolk’s character could offer a sharp contrast to the hospital’s current leadership vacuum.

Premiere Date and Where to Catch Up on ‘Grey’s Anatomy’

All previous seasons are available to watch on both Netflix and Hulu. That makes a quick refresher easy for anyone who has drifted away. The most recent episodes are the ones that matter most for the premiere.

Several relationship threads are worth rewatching. Lucas admitted he still has feelings for Simone, even though things have become serious between Simone and Wes. Amelia rushed into a hookup with Cass, and Toni then arrived at her door wanting to give the relationship a real chance.

The workplace storylines carry more weight. Blue implanted an experimental treatment into a terminal cancer patient, Richard fired him, and Bailey was suspended for not reporting what she knew. Jules and Winston have also not told HR about their relationship, and Ben caught them leaving an on call room.

The renewal also reflects a series with staying power. The show averaged 2.213 million viewers per episode and logged 40.9 billion minutes streamed across platforms in 2025. With Owen and Teddy gone and Bailey’s job in doubt, does Grey Sloan Memorial need an outsider like Seth Norton to steady it, or should the next chief come from within the hospital’s own halls?
