‘Chicago Med’ Cast Shake-Up Reshapes Gaffney as the New Season Opens

‘Chicago Med’ returned this fall on NBC, with its new season airing at 8 p.m. Eastern on Wednesdays. The premiere date was Wednesday, October 7. The return comes with one notable absence that fans began to suspect weeks before the episode aired.

Ashlei Sharpe Chestnut, who plays Dr. Naomi Howard, is leaving ‘Chicago Med’ after three seasons. The ‘Chicago Med’ cast changes arrive at a time when the wider One Chicago franchise is also adjusting to a wave of departures, which makes this exit part of a larger story about how NBC’s Windy City dramas are evolving.

Chicago Med Cast Changes and the Naomi Howard Exit

Sharpe Chestnut confirmed her exit on TikTok and will not return for the new season. A fan had asked in a comment whether she would be back, and she answered with a simple no. When fans voiced their disappointment, she added that she would have loved to come back.

Sharpe Chestnut joined the show in Season 9 as a student doctor in the emergency department of Gaffney Chicago Medical Center. She appeared in a total of 28 episodes, including the Season 11 finale. She was never promoted to series regular, and remained a recurring presence throughout her tenure.

There was little warning that the exit was coming, and her absence from the promotional poster for the new season was one of the first signs that something had changed. That detail gave viewers an early clue, even though the show itself had given no on screen indication that Naomi’s story was ending.

What the Chicago Med Showrunner Has Said

Early coverage noted that Sharpe Chestnut did not explain why she was stepping away. More recent reporting has filled in part of that picture. Showrunner Allen MacDonald told Give Me My Remote that the plan had been for Naomi to return, but that Sharpe Chestnut elected to move on because she has a lot of opportunities.

That account makes clear the exit was not driven by budget constraints or a creative decision, which are the reasons often behind departures on these shows. It also suggests the door was not closed by the writers’ room, which matters for a character who was still an active part of the ensemble.

Before the news broke, MacDonald had teased a bump ahead for Naomi and Dr. Frost, and he did not mention Naomi at all when answering questions about the season. Looking back, that silence reads as a quiet hint, and it helps explain why some viewers suspected the worst before any confirmation arrived.

How Naomi and Frost Fit Into the Story

Naomi and Frost finally became a couple in the previous season after a long stretch of will they or won’t they storytelling. The Season 11 finale left the two still dating, so her departure was not set up in advance.

Because her exit is expected to happen off camera, viewers are not likely to get a proper sendoff for the character. One Chicago Center suggested it will be explained through dialogue, and voiced hope that the writers address it in a meaningful way.

The promo for the second episode, set for October 14, shows Dr. Frost hooking up with a new love interest. That storyline suggests the series is already moving Frost’s personal life forward, and how it handles the absence of Naomi will shape how that new chapter lands.

One Chicago Departures and the Main Cast

Other One Chicago exits include Joe Miñoso, Dermot Mulroney and Chicago Fire showrunner Andrea Newman. On Chicago P.D., original cast member LaRoyce Hawkins is also leaving. Taken together, the changes make this a busy stretch for the franchise.

The picture at ‘Chicago Med’ is more stable than it might first appear. The show’s main cast of series regulars remains unchanged, and apart from Marlyne Barrett leaving after Season 10, there have been no further official exits or additions among them.

The series premiered in 2015 and follows the doctors and nurses of the emergency department at Gaffney Chicago Medical Center. After so many years, turnover is part of the rhythm, and the show has a track record of absorbing departures while keeping its ensemble intact. With Naomi gone and Frost already moving on, do you think the writers owe viewers a real goodbye for her story, or is a quiet off screen exit the right way to close it?
