Finale Recap and the Hospital Lockdown

The finale continued a terrifying lockdown after a violent prison riot sent several injured inmates and guards to the hospital. Police cleared the hospital floor by floor, which left labor and delivery isolated for far too long as the danger spread through the building.

The storylines going in included the revelation that Theo might be a sociopath, Dean preparing to confess his love for Hannah, and Frost learning that his father was in hospice. The lockdown forced all of those threads to collide in a single night.

The finale also revealed that Dupere was never trying to murder Isaac. Charles also learned that a patient named Scott was Gio, the young man who called the helpline earlier in the season and whom Charles suspected had killed himself.

Hannah and Archer Welcome a Daughter

The hour opened with Hannah attempting to guide her patient through childbirth while she experienced labor herself. She eventually had to tag in Jacob to take over as her own contractions intensified.

Archer spent most of the episode away from the delivery floor. A prisoner had fled his hospital bed, seized a firearm and taken Archer hostage, telling him to follow every command if he wanted to see his child. Archer, an experienced veteran, took down the gunman and ran to the delivery room just in time.

Hannah named the baby Mabel, and Archer then made his big confession. As TVLine reported, the finale prompted a conversation with Jessy Schram and showrunner Allen MacDonald about what it called Hannah’s ambiguous response to that confession. Hannah said the stress of the baby had kept her from processing his feelings.

Lenox and Ripley Face a Terminal Diagnosis

The finale included mutual declarations of love between Lenox and Ripley as they confront her terminal diagnosis. The episode picked up in the fallout of her telling Ripley she loves him. The scenes gave a quieter counterweight to the lockdown plot.

Lenox also told her brother about her prion disease diagnosis. He reacted in an extremely emotional, almost violent way, but the two ultimately embraced and their relationship appears stronger now that she has come clean.

The diagnosis reframes a season in which Lenox’s storyline was already under strain. Earlier in the season she was pistol whipped by an abusive husband while searching for a former patient. The finale suggests her story is now about time and honesty more than survival.

Frost’s Father and the Fallout for Gaffney

After being moved into hospice care, Frost’s father James died, but not before Frost made amends and said goodbye. Frost curled up beside him and told him it was all right to let go. It stands as one of the most restrained scenes in the hour.

Work problems also lingered at the hospital. Goodwin’s job was put in jeopardy after she confronted Rabari’s manipulation, which sets up conflict for the next season. Charles’s job also remained in jeopardy.

The first episode of the next season arrives Wednesday, October 7, 2026, at 8:00 PM ET, with new episodes streaming the next day on Peacock.
