It brought back Scott Foley’s Sean Kelly, a marine biologist and Elliot’s former boyfriend, who crosses paths with J.D. while he is on a date with his new girlfriend. This ‘Scrubs’ recap walks through what happened and why the ending matters for Elliot.

Sean is swimming in a tank with sharks when he spots J.D., and he climbs out to greet Elliot’s former husband and his new girlfriend. The meeting is awkward, and it gives the episode its comic setup. J.D. clearly did not expect to see this particular name from Elliot’s past in a shark tank.

J.D. then fills Charlie in on the history, explaining that Sean is Elliot’s former boyfriend, that J.D. sabotaged the relationship, and that he may have ruined Sean’s life in the process. Sean has his own reservations and asks J.D. whether he still has feelings for Elliot. That question hangs over the rest of the episode.

According to Decider, Carla is desperate to find Elliot someone new now that she is single again, and she even suggests a doctor from a nearby hospital. The suggestion goes nowhere, and the story steers back toward Sean.

Sean and Elliot Reunion Ending Explained

Elliot is not quick to say yes. She hesitates over Sean’s dinner invitation, then turns up in her pajamas and mouth guard to tell him the timing is wrong because she wants to focus on her career. That fits the direction her story took earlier in the season.

In the premiere, Elliot began reassessing what she wants from the next phase of her career with encouragement from Tosh. Her reluctance here reads as a continuation of that thread and not a throwaway joke. She is trying to decide what comes next before anyone else decides it for her.

The turning point comes from J.D., who has found happiness again with Charlie and wants the same for Elliot. She takes that to heart, visits Sean at the aquarium, agrees to give the relationship another chance, and the two seal the reunion with a kiss. The ending is warm, and it leaves the door open for a longer arc.

MovieWeb argues that the episode lets J.D. finally move past his old jealousy of Sean. That reading adds weight to a scene that could otherwise pass as a simple romantic payoff.

Scott Foley’s Return as Sean Kelly

Sean appeared in a total of twelve episodes between 2002 and 2009. J.D. had already tried to reunite him with Elliot once before, after his own relationship with her fell apart, and Elliot turned Sean away because she felt guilty about calling things off so she could settle down with J.D. The new episode picks up that unfinished business.

In an interview with TVLine, showrunner Aseem Batra broke down the premiere, and the outlet noted that Foley would return in this episode. The same report pointed out that J.D. and Sean did not like each other the first time around. That makes J.D.’s role as matchmaker a more interesting choice.

Foley is joined by Michael James Scott as Nurse Dubois, John Higgins as Grant Summers and David Gridley as Dr. Blake Lewis. The supporting cast keeps the hospital setting alive while the main story plays out at the aquarium.

What the Episode Leaves Out About Sean and Kim

The last time audiences saw Sean, he was dating J.D.’s former girlfriend Kim, played by Elizabeth Banks, and J.D. does not bring that up when he explains the history to Charlie. Kim is the mother of J.D.’s firstborn son, Sam. The omission is hard to miss for longtime viewers.

TVLine frames the decision to skip over that storyline as deliberate. The writers appear to be keeping the focus on where each character stands now and not on the loose threads of the original run.

The next episode, “My Chief,” is written by Mike Royce and is scheduled for October 14, as the ten-episode second season continues. Whether J.D. was right to play matchmaker for Elliot and Sean a second time is a fair debate, and anyone who watched that aquarium kiss may have strong feelings to share in the comments.
