‘Chicago Med’ opened its new season on Wednesday, October 7, with an episode titled “Shots Fired.” The premiere picks up three months after the previous finale and drops a hospital already short on staff into a mass casualty event.

Goodwin has taken a voluntary leave, and Mallory Sloan, played by Rebecca Breeds, is serving as executive director. Sloan has received a cool welcome because of budget cuts and staff burnout. That friction is clear when Archer asks for more help in the emergency department and Sloan turns him down.

Asher stops by the hospital with Mabel and explains her shared parenting arrangement to colleagues. She also tells Archer she will be on maternity leave for a few weeks before returning to work. It is a quieter personal beat that sits alongside the larger crisis around Goodwin.

The Wedding Shooting That Strained the Emergency Department

A shooting at a wedding injures at least a dozen people, including the bride, Tamar. Doctors discover she is pregnant and that the bullet has damaged her uterus, and Asher agrees to lead the surgery. Tamar later tells police that her former partner, Henry, was the shooter and could not accept their breakup.

The groom, Dexter, is missing for much of the episode and turns out to be uninjured. He tells Charles he never drove to the ceremony because he was unsure of his decision, and his mother had convinced him that Tamar was after his money. By the end, Dexter apologizes to Tamar and stops his mother from entering her room.

Frost treats an eight year old patient named Dakota who cannot see. Doctors find nothing wrong with her eyes and conclude her blindness is a trauma response. Bruno, the musician who brought her in and encouraged her, is then killed when a car crashes through the emergency department doors.

The driver is Henry, and Ripley is left to tell Frost what happened. A mix up is later cleared when Dakota’s mother is found to be alive. It is a heavy sequence that gives the premiere its emotional weight before the final reveal.

Ending Explained as Doris Confesses to the HIPAA Leak

Earlier in the episode, Miranda offered Goodwin a deal that would require her to step down indefinitely. Goodwin refused and vowed to uncover the truth herself. Then Nurse Doris arrived at Goodwin’s home and said, “It was me, Miss Goodwin,” admitting she had leaked Rabari’s medical file.

In an interview with <a href=”https://www.tvline.com/2280697/chicago-med-recap-season-12-premiere-ending-explained/”>TVLine</a>, showrunner Allen MacDonald said the leak was one of the options on the table when he wrote the previous finale. The writers settled on Doris because it subverts expectations and avoids the simpler answer that Goodwin did it.

MacDonald explained that Doris carries guilt that grows as the hospital struggles without Goodwin, and the moment of silence for the shooting victims becomes her turning point. He also cautioned viewers with a laugh, saying “I would not take anything you see in the premiere at complete face value.”

Lenox Returns and What Comes Next for Goodwin

Sarah Ramos surprises viewers as Dr. Caitlin Lenox, who was not expected to appear for the first five episodes because Ramos is on maternity leave. Lenox shows up on a video call with Ripley and reveals she is in Zurich for a prion disease drug trial. MacDonald said the appearance was planned all along, and the scene was filmed when Ramos returned for the sixth episode.

MacDonald described a real and open connection between Lenox and Ripley, who narrowly avoids the car that kills his new friend. He called the near miss a wake up call for Ripley.

Goodwin still faces a major fight to get her job back, and MacDonald has teased that she will have a very unique way of fighting back in the second episode. That episode, titled “Third Rails,” is scheduled for October 14. New episodes air Wednesdays on NBC, with streaming on Peacock the next day.
