ABC has given a series order to a still untitled medical drama set in a rural West Texas town, created by Shonda Rhimes and current ‘Grey’s Anatomy’ showrunner Meg Marinis. The announcement arrived after ‘Grey’s Anatomy’ had passed twenty two seasons and more than 450 episodes, and it gives the Texas spinoff a long runway of franchise history to draw on.

The move also signals a shift in how the franchise grows. It will be the first show in the ‘Grey’s Anatomy’ universe not set in a West Coast city, which makes the Texas spinoff a notable change of scenery for a brand built on Seattle and Los Angeles hospitals.

Texas Spinoff Premiere Window and Episode Plans

The spinoff is expected to premiere in midseason, in 2027. ABC has described it as part of the 2026 to 2027 broadcast season. That timing fits a scheduling plan in which ‘Grey’s Anatomy’ airs in the fall and the spinoff takes over the slot at midseason.

Fans hoping for specifics will have to wait. The number of episodes in the series order has not yet been determined. The title is also still under wraps, and ABC has not yet shared a firm premiere date.

The mothership series is not going anywhere in the meantime. Fans can keep up with Season 23 of ‘Grey’s Anatomy’ on ABC while the new series moves through development.

Creative Team Behind the ‘Grey’s Anatomy’ Spinoff

The series is produced by Shondaland and 20th Television, with Rhimes and Marinis serving as writers and executive producers alongside Betsy Beers and Ellen Pompeo. Pompeo’s involvement as a producer keeps the franchise’s original lead close to the project, even though her role on screen has not been addressed.

Rhimes is returning to the writing side of the franchise in a meaningful way. Reporting on the order noted that the spinoff sees Rhimes return to a ‘Grey’s Anatomy’ writers’ room as a writer or co-writer for the first time since 2011.

Marinis brings a deep personal connection to the material. She grew up in Houston and has worked on the original ‘Grey’s Anatomy.’ She interned at Universal before landing a production assistant role on the series in 2006, and has woven Texas experiences into the flagship series in recent years.

Her reaction to the order was enthusiastic. She said she was incredibly excited to expand the ‘Grey’s Anatomy’ universe.

Rural West Texas Setting and Medical Drama Premise

The official logline calls the series an edgy drama about a team at a West Texas rural medical center, and describes it as the last chance for care before miles of nowhere. That premise places the story far from the well staffed urban hospitals that defined earlier entries in the franchise.

The setting carries real world weight. West Texas will likely introduce new challenges, from geographic isolation to healthcare scarcity, exposing the realities of rural medicine in America. Rural healthcare access remains a growing issue across the state, which makes the backdrop particularly timely.

It also marks a structural departure. The upcoming series will be the first ‘Grey’s Anatomy’ spinoff that does not focus on a character from the original show. The earlier spinoffs, including ‘Private Practice,’ ‘Station 19’ and ‘Grey’s Anatomy B Team,’ were all set on the West Coast and featured at least one character from the original series.

Cast Questions and Returning ‘Grey’s Anatomy’ Characters

On casting, the picture is still empty. No cast members have been announced so far. Early reports also left open how closely the series would tie into the original, and connections between the spinoff and the original series remained unclear, including whether Meredith Grey or other established characters would appear.

Marinis has since offered some clarity. She confirmed that ‘Grey’s Anatomy’ characters will show up in some form on the spinoff, and that the series is actively in development. She has not said which characters, which leaves room for speculation.

One name already draws attention. Levi Schmitt moved to Texas with his partner when he left ‘Grey’s Anatomy’ in Season 21, though his return has not been confirmed. Former cast members are also making their interest known, since Leven Rambin said in a video that she is in Texas right now and that the spinoff needs to happen.

With a fresh ensemble expected to anchor the series and only a few familiar faces likely to cross over, which ‘Grey’s Anatomy’ character would make the most natural fit for a remote West Texas medical center, and who would you like to see make the trip from Grey Sloan Memorial?
