Ui Ui is very close to his sister and is devoted to her
Mei Mei’s younger brother, Ui Ui, resembles her in appearance and hair type, even though he is still a young child. Given how much his personality resembles Mei Mei’s, it’s amusing to see the two of them interact with each other and with others. They appear to be very devoted to one another, yet they also have distinct ways of speaking and acting with other people. Ui Ui is incredibly devoted to his sister since he holds her in such high respect and appears to believe that she is the greatest and most unique person in the world.
When Ui Ui claimed that Satoru Gojo saw Mei Mei as a “bench player,” the boy immediately belittled him despite Mei Mei’s insistence that Gojo was unique. Ui Ui did not care; all he did was claim that Mei Mei was unique as well. Itadori was dumbfounded by their conversation, unable to fully comprehend that aspect of their relationship—especially because he felt like a worthless “commoner” in their presence.
He appears to take pleasure in Mei Mei’s compliments as well, despite the fact that he occasionally pretends not to believe her, just to receive more of them. Even though Mei Mei is more powerful than him, he still views herself as her protector. For this reason, he urged Itadori to remain silent when Mei Mei was employing her Black Bird Manipulation method, and he became irritated when Mei Mei informed him that Yuji may speak because it did not upset her. He is by her side at all times and is utterly devoted to her.

We have given you a relatively precise overview of the relationship between Ui Ui and Mei Mei, but the gist of it is something else completely. Namely, their whole relationship could be consensed in the way Ui Ui addresses his sister, which – as those who know Japanese and about Japanese society – says it all. If you were wondering who “Nee-sama” is, we can confirm that it is Mei Mei.
Namely, in Japanese, the term for one’s older sister is “onee” or “ane” (the Kanji is [お]姉), but this is a generic term. So, if you hear someone addressing someone as “Onee-san”, which is the most common expression, this means that that person is their older sister. Since we know that Mei Mei is Ui Ui’s older sister, it makes sense that he calls her like that.
But why “-sama”? In Japanese, you can address a girl as “-san” (the most common and polite way), “-chan” (usually reserved for younger girls or as a term of endearment), and even “-sama” (which is a sign of respect and is usually used by those who are in an inferior position to someone, e.g., a younger person when talking to an older one). So, knowing how much he holds her dear, it is no surprise that Ui Ui actually addresses his sister as “-sama”, so this should not come as a surprise to anyone, but we have still explained it, and now you know everything about the issue.
